RPD support와 stability 늘리기

RPD 성공을 위한 3가지 중요한 요소는 support, stability, retention이다. 가철성 의치 특히 distal extension base가 존재하는 경우에 치아와 edentulous ridge에서 적당한 support와 stability를 얻는 것은 아프지 않는 가철성의치의 핵심이다.

가철성의치의 support와 stability를 증가시키는 방법으로는 edentulous ridge에서의 functional impression, abutment의 splinting을 통한 치아의 support 및 stability 증가, clasp가 아닌 attachment(double crown)의 사용, 전략적인 위치에 implant를 식립하는 방법 등을 생각해 볼 수 있다.

이번 강의에서는 기존 교과서에서는 정리되지 않은 clasp RPD에서의 abutment의 상태에 따른 splinting 기준, abutment의 상태(C/R ratio)가 좋지 못한 경우에 splinting할 경우의 문제점, C/R ratio가 좋지 못한 경우와 소수잔존치에서 support와 stability를 증가시키는 double crown RPD의 적응증과 biomechanics, implant combined RPD의 적용하는 attachment의 종류와 그 예후를 살펴보고자 한다.
